I understand that by attending this mission trip that I will abide by the rules of Six Points Church when it comes to proper behaviors on the trip (no smoking, no drinking, appropriate language and behaviors toward others) as laid out in the Community Standards Agreement I will receive in full at our team meetings. I am at least 12 years old or have completed the 5th grade (whichever comes first) and if I am a child that a parent or guardian will attend with me. I agree that the cost of the trip is $250 per person and that I will work with team efforts to fundraise for myself and/or others. I agree that I need to be in good health in order to fully participate in the mission projects. I understand that this is not a vacation, but an opportunity to serve others. My attitude will reflect this. There will be several team meetings to help us prepare for this service opportunity and I will make those a priority to attend.
